Job Summary

Coordinate healthcare services for Tejas Health Care (THC) patients in order to ensure a holistic, whole-person approach is taken towards providing a health home for all eligible patients. Health home is defined as “person-centered systems of care that facilitate access to and coordination of the full array of primary and acute physical health services, behavioral health care, and long -term community-based services and supports.” In addition the individual must manage the core competencies of the position to include service coordination, teaching and education, advocacy, interpersonal skills, and record-keeping.

Duties and Responsibilities

•Identify and engage care coordination patients at Tejas Health Care that have been identified as having at a minimum of at least two (2) chronic health conditions such as but not limited to asthma, diabetes, heart disease, obesity, hypertension, substance abuse disorder, or behavioral health condition; one (1) chronic condition and be at risk for another; or one (1) serious and persistent mental health condition.

•Serve as the system navigator and point of contact for patients and families for asking questions and raising concerns.

•Participate as a member of a multi-disciplinary team including representation from all Tejas Health Care services who are working toward the development and implementation of health home services.

•Coordinate with the Tejas Health Care team as needed to ensure that high risk, high volume problem prone diseases as well as preventive and health promotion needs are addressed as part of the holistic, whole-person, culturally-sensitive healthcare approach.

•Serve as the liaison between the behavioral health provider and patient to ensure that the phrenological treatment is seamless and uninterrupted.

•Teach and assist Tejas Health Care patients to better manage their diseases using realistic approaches to ensure compliance.

•Provide teaching to patients/families on an as needed basis and assist in problem solving potential issues related to healthcare.

•Negotiate on behalf of the patient, as needed, with the healthcare treatment team and the patients themselves by creating reasonable goals, clarifying issues while remaining flexible and consider culturally-sensitive and holistic alternatives.

•Completes interagency and all necessary paperwork to include interaction with primary care providers that see our patients outside of Tejas Health Care.

•Communicates effectively with other staff members, representatives of other agencies and the public.

•Adheres to the policies and procedures of Tejas Health Care.

•Performs other duties as assigned.

Knowledge, Skills and Abilities

Successfully completed training and licensure as a Licensed Vocational Nurse in the State of Texas and has current CPR certification.

Bi-lingual desirable

Possesses good organizational skills and is self-directed.

Demonstrates flexibility and emotional control in handling difficult or stressful situations.

Proficient with computer work and an understanding of medical computer systems.

Must be able to tolerate rapid fluctuations in workload

Must be able to lift, bend, kneel, reach, push and pull loads and stand for long periods of time.

Special Requirements

The employee must be able to lift and/or move more than 50 pounds. Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye/hand coordination. Requires sitting, standing and walking for extensive periods of time. Requires working under stressful conditions or working irregular hours. Requires frequent exposure to communicable diseases, body fluids, toxic substances, medicinal preparations and other conditions common to a clinic environment. The employee frequently is required to reach with hands and arms. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Job Summary

Provide clerical duties at point of entry for assigned clinics through scheduling, reception, records maintenance, coordination and general assistance for administrative support activities in clinic.

Responsibilities

•Greets and assists clients of and visitors to Tejas Health Care clinics

•Answers, screens and directs incoming phone calls

•Schedules and confirms appointments

•Verifies insurance for current status and coverage

•Assists clients in completing necessary paperwork

•Contacts patients regarding No Show visits

•Assists patients with balances with making a payment plan or works with them and the billing department to resolve payment issues

•Accurately enters client information into computer data bases for program eligibility

•Maintains forms and basic information on patient charts and electronic databases

•Creates charts for new patients

•Performs clerical duties including mail delivery faxing, copying and scanning

•Assembles packets of information for clients

•Assist with other clinical and clerical duties as required

Required Skills and Abilities

•Able to understand and follow directions

•Capable of communicating effectively with patients and the health care team, including the ability to explain providers’ instructions to patients

•Knowledge of customer service concepts and practice

•Able to establish and maintain effective working relationships with the public and health care team

•Must respect the confidential nature of health information

•Working knowledge of standard concepts, practices and procedures

•Capable of following established departmental procedures

•Able to work efficiently and cope with emergency situations

•Capable of using experience and judgment to plan and accomplish goals

•Good computer skills and a working knowledge of Microsoft Office

•Able to read and interpret documents such as charts, safety rules, operating and maintenance instructions and procedure manuals. Ability to write routine reports and correspondence

•Professional manner and appearance

•Emotional and physical health sufficient to meet the demands of the position

Education and Experience

•Requires a minimum of a high school diploma or GED

•6 months to 2 years of experience as a Medical or Dental Clerk preferred

•Able to speak, read, and write Spanish preferred

Job Summary

Provide professional substance abuse and mental health counseling to patients. An individual must be able to perform each essential duty, including mental and physical demands and OSHA requirements satisfactorily.

Duties and Responsibilities

1.Provides clinical services.

a.Coordinates counseling services with Behavioral Health Clinician; coordinates medication management with Psychiatrist, psychologist and medical providers.

b.Develop and implement individualized treatment plans with current diagnosis.

c.Provide individual, family, and group therapy for patients.

d.Conduct mental health and substance abuse assessments and diagnosis.

e.Crisis intervention – walk in, phone, or medical department referral

f.Chart clinical date for diagnosis, treatment, referral, accountability and research.

Knowledge, Skills and Abilities

Required

•Knowledge of advanced principles of health promotion, prevention and motivation

•Knowledge of protocols in general and an ability to quickly gain specific knowledge of Tejas Health Care’s current operational protocols

•Knowledge of medical terminology and appropriate abbreviations

•Demonstrates knowledge of the Texas Medical Practice Act

•Ability to document, with clarity, all information relevant to a patient’s needs

•Ability to effectively present information in small group situations to patients, Tejas staff, and the general public

•Ability to use office equipment including but not limited to computers, fax machines, telephones and copiers

Preferred

•Spanish speaking and writing skills

Credentials and Experience

Required

•Master’s Degree in Behavioral Health (Social Work, Psychology, Sociology or related behavioral health field)

•One year directly-related field experience.

•Hold a license to practice i.e., LPC, LCSW, LMFT, LBP or under supervision
